Building a simple PHP form by dheep30

This is an assignment, which deals with TNM staging of Breast cancer. Would like to have a php form created, where any one could just type the values of T,N and M. By clicking the submit button, the form should show the stage of the disease. (eg, T1N1M0 = stage IIA disease; T3N1M1 - stage IV disease and so on).
